Remember to take a  PCQ Horse Health Declaration to all events at you attend. This is a part of Biosecurity and is for the safety of all riders and horses.

Horse Health Declaration Forms need to be presented upon arrival at the Event.  A good tip is to copy the Horse Health Declaration Form from the PCQ website ( and fill in your name, address, Property Idenditification Code Number (PIC NO:) – this number is available from the Queensland Department of Primary Industry, and copy several times.  Place these forms in a folder along with performance cards, and membership card and that way you will always have it with you.

It is NOT up to the event organiser to have this form for you, as a horse owner IT IS YOUR RESPONSIBILITY.  Some events insist no form no ride.  Microsoft Word – PCAQ Revised (May2014) Horse Health Declaration
